4.) Items to Bring with you to Our Office
Please remember to bring your insurance card and photo identification, and arrive 15 minutes early in order to complete any additional paperwork before seeing your provider. You may also want to bring a pair of shorts and a t-shirt to wear for your physical exam. If you don’t, a paper gown will be provided. Please present your completed forms to the receptionist upon your arrival. If you are more than 15 minutes late for your appointment you may be asked to reschedule. If your insurance company requires a referral, please make sure that you have this in place before the day of your visit. Without a valid referral, you may be asked to reschedule.
Before your first visit, please ask your referring physician to fax a copy of any blood tests or x-ray reports done in the last year to our office. Our office fax number is (716) 675-2191.
